Parental Consent
The Dance Process (“TDP”) does not allow students under the age of 18 to participate in online lessons without the consent of their responsible adult (e.g., parent or legal guardian).
By signing this form, you agree to give your minor child/children permission to participate in lessons delivered via the TDP Application. A responsible adult must ensure that their child/children will be appropriately supervised while participating. A responsible adult must further ensure that the environment where their child/children will be engaging with the activity is safe, that the child/children wear appropriate clothing/footwear for the space in which they are dancing, and that there are no dangerous or hazardous obstacles.
Please ensure that you have read and understood the TDP Privacy Policy and Terms of Use.
By executing the below, I agree that I have read and understand the TDP Privacy Policy and Terms of Use and agree to be bound by its terms. I further confirm that my minor child/children are permitted to use the TDP Services (as defined in the Privacy Policy and Terms of Use) and confirm my understanding and agreement to the information and terms as outlined above.
